After most tests have been returned, I ask for a "fix and reflect". What this means is that - on a seperate sheet of paper - the student should FIX the problems missed, and then REFLECT on the root causes of that particular mistake by writing a sentence or short paragraph identifying the root cause. There are essentially 4 root causes of all math errors:

Concept - the student didn't understand the concept

Mechanics - the student didn't practice the mechanics of solving the type of problem enough to get the procedure down pat.

Precision - missed a detail such as a negative sign, misread instructions, handwriting caused a mistake to be made, etc.

State of Mind - student was too sleepy or sick the day of the lecture, the test, or during study time.

Due Thursday, Oct 1:

Algebra I - The worksheet 1.3 and 1.4
Pre-Calc - Read Section 4.1
Trig - Section 2.1, 1 to 90 odds
Calculus - Section 3.5
